Important Information
All first time new patient appoitments must be scheduled for an in person office visit. Telehealth appointments are not an available option if this is your first time visiting Fairway Behavioral Health
Before your first appointment as a new patient, you must fill out all of the forms linked below : Consent to Treat, Telehealth Consultation Agreement, ID and Insurance Coverage, New Patient Intake Form, and the Information Release. Failure to fill out the required documentation may result in a delay with your appointment.
